Required Qualifications

• Active and in good standing state LVN or LPN license. (experience)
• Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses). (experience)
• Knowledge of medical terminology and clinical documentation standards. (experience)
• Experience with phlebotomy, vaccine administration, and point-of-care testing. (experience)
• Excellent communication and organizational skills. (experience)
• Experience working in a clinical setting with diverse cultural dynamics among staff and patients. (experience)
• Proven ability to manage sensitive information ethically and responsibly. (experience)
• Strong problem-solving, critical thinking, and decision-making skills. (experience)

Responsibilities

• Administer oral, injectable, and topical medications.
• Evaluate complex patient conditions, monitor pain levels, and document treatment responses.
• Apply advanced knowledge of vital sign monitoring to identify and record abnormalities in blood pressure, heart rate, respiratory rate, and temperature.
• Perform routine assessments to monitor patient health and ensure overall well-being.
• Manage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management.
• Conduct medication reviews to identify potential side effects and drug interactions.
• Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function.
• Accurately document patient medical history—including illnesses, medications, allergies, surgeries, and family history—using electronic health records.
• Use your excellent communication skills to interact with patients, families and other healthcare professionals

• general: This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
