The Care Extender / Peer Support Specialist - Foster Care is responsible for engaging with community partners and builds strategy with local stakeholders; providing education, strengthening relationships, and helping resolve case or broader systems issues. Supports care coordination teams and connect members and caregivers with those who can provide insight and expertise to assist in overcoming challenges.How you will make an impact:Identify opportunities for engagement of members and their caregivers and families to share with care management team.Partners with peer support services in the community to create referral pathways and links members to appropriate community peer support services.Support care manager as they advocate for foster parents and other caregivers, addressing their needs or challenges with various agencies and systems.
