Specialty Pharmacist, Hemophilia (Remote) Candidates must reside in and hold an active pharmacist license in FL, TX, MO, CA, or MI.As a Specialty Pharmacist supporting patients with hemophilia, you'll play a vital role in helping individuals manage a complex chronic condition by ensuring access to specialized therapies, optimizing treatment outcomes, and providing compassionate patient support.What We're Looking for:A licensed pharmacist with hemophilia pharmacy experience.Must reside in and maintain an active pharmacist license in one of the following states: Florida, Texas, Missouri, California, or Michigan.Strong verbal and written communication skills.Excellent problem-solving and critical-thinking abilities.Exceptional organizational and time management skills.A collaborative, adaptable professional who thrives in a fast-paced environment.A patient-focused commitment to delivering high-quality specialty care.Job Specific Details:Ensure patients receive the right medication, know how to use their medication, and address/resolve drug therapy issues prior to dispensing.Review and enter prescription orders accurately and efficiently.Provide patient counseling and education regarding medications and treatment plans.Verify prescription data entry for completeness and accuracy.Conduct drug utilization reviews to identify and resolve potential therapy-related concerns.Partner with clinics to clarify and obtain prescriptionsWork Schedule: 10:30AM - 7PM if you are (Eastern/EST), 9:30AM - 6PM if you are (Central / CST) or 7:30AM - 4PM if you are (Pacific / PST) Monday through Friday. Some holidays are required when business is open.Position Purpose:Coordinate and assist in managing and participating in pharmacy services, including dispensing of medications, quality assurance and clinical monitoring activities.Participate in decisions regarding the pharmaceutical services within assigned scope including:Assessing the patient's general pharmaceutical needsPlanning for the pharmaceutical services provisionParticipating in the overall plan of careParticipating in the overall quality assessment and improvement planReceive physician prescription orders and evaluate the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stabilitySupervise the filling, receiving, compounding, and dispensing activities involved in providing finished product to the patientEnsure that the patient chart is complete for allergies, height, weight, current and past medical history, clinical laboratory results and concurrent medications (including over-the-counter or home remedy medications)Ensure that when necessary, laboratory tests have been ordered and the results are obtained and discussed with the nurse and the physicianProvide patient counseling, education, and monitoringEnsure safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patientDetermine remaining doses on hand to manage inventory and avoid wasteProvide clinical interventionsAssess responses to therapy, patient compliance, and ongoing review of drug regimens; report and manage adverse eventsAssist the physician, nurse and patient with clinical and technical supportDocument all communication with physicians, nurses and patients in the patient chartAssist in the maintenance and upkeep of all necessary paperwork required by state and federal agencies as well as the Joint CommissionAssist the Pharmacy Manager in implementing new proceduresAssist in the training and supervision of Pharmacy Technicians, Pharmacy Interns and other pharmacy personnelParticipate in Performance Improvement ProjectsAssist in providing in-service programs to nursing and pharmacy personnel on any aspect of drug therapyAttend and/or complete education and training programsReview literature and other materials pertinent to the practice of pharmacyComplete all competency/skills assessment requirementsParticipate in on-call schedulePerform other tasks as determined by the Pharmacist-In-ChargeComply with all policies and standardsEducation/Experience: Bachelor’s degree in pharmacy and 3+ years of mail order, retail or hospital pharmacy experience OR Pharm D. Experience in Hemophilia, highly preferred. Licenses/Certifications: Current state’s Pharmacist license and license at other states as required.
