Heart Failure Specialist Pharmacist and Independent PrescriberATTR-CM Amyloidosis Support ProgrammePart-time – 3 days a week (remote with 1 day a week onsite in London)We are seeking an experienced Heart Failure Specialist Pharmacist and active Independent Prescriber to support an innovative ATTR-CM Amyloidosis Support Programme delivered in partnership with the National Amyloidosis Centre at the Royal Free Hospital.This is an excellent opportunity for a GPhC-registered pharmacist with specialist heart failure experience, cardiovascular independent prescribing expertise and confidence delivering remote clinical consultations. You will work closely with the National Amyloidosis Centre, local cardiology teams, primary care, community services and homecare providers to support high-quality, patient-centred care.What you will be doingAs a Heart Failure Specialist Pharmacist, you will deliver specialist clinical pharmacy and independent prescribing support for patients with ATTR-CM. Required Qualifications
